Extra Service Hours and Feedback

Use this form to report service hours for a non-profit organization outside of Key Club. After filling out the form, have the organization sign and date it, then email it to Secretaries Naomi Hsu (816120@seq.org) and Cricket Lee (813543@seq.org), and Kiwanis Advisor Rosie Jimenez (rjimenez2@sbcglobal.net) by the end of the month that the service is rendered.
